What is Capsulitis? Capsulitis is a condition involving inflammation of the joint capsule – typically affecting the ball of the foot, beneath the base of the second toe. The joint capsule helps stabilise the joint, and when it becomes irritated or inflamed, it can lead to pain, swelling, and instability. In more advanced cases, the toe may begin to drift upward or sideways, potentially leading to deformities like hammertoe.
What causes Capsulitis? Capsulitis usually results from abnormal foot mechanics that place excess pressure on the ball of the foot. Contributing factors may include:
- A longer second toe (often seen in Morton’s foot structure)
- High arches or flat feet
- Unsupportive or tight footwear
- Repetitive stress activities like running or dancing
- Thinning of the fat pad on the ball of the foot with age
- Underlying joint conditions such as osteoarthritis or rheumatoid arthritis
These mechanical issues can strain the metatarsophalangeal (MTP) joint, causing inflammation in the joint capsule.
How is Capsulitis treated? Early diagnosis and intervention are essential to prevent progression. At Total Foot Health, we provide tailored care that may include:
- Activity modification to reduce pressure on the affected joint
- Bespoke orthotics to correct mechanical faults and offload pressure from the joint
- Supportive footwear advice to improve stability and cushioning
- Exercise programmes to strengthen the foot (if deemed appropriate)
- Referral to a rehabilitation specialist for ongoing care
We strongly advise against self-diagnosing or attempting taping or manipulation without professional guidance. Capsulitis shares symptoms with other conditions such as Morton’s Neuroma and can only be accurately diagnosed by a specialist podiatrist through physical examination and, in some cases, ultrasound imaging.
Total Foot Health is an RCOP-accredited practice. We specialise in identifying and treating mechanical foot conditions through evidence-based, personalised care.
